TEAM MEMBERS FOR SUMMER LEAGUE
Each person may select to join with two other women and two other men. This will allow as many as five people to join as a mixed group of three women and two men or vice versa. Please endeavor to have those that you select also indicate your name in their selections. We will attempt to accommodate your requests, and will be more able to do so, if all members of your group write each other down. Please feel comfortable signing up just as an individual, as many of the participants do, and in fact, these people usually have the most fun.

PAYMENT AND REGISTRATION
You may sign up for the Summer league, the Frostbite league, or both. Please make sure to fill in the appropriate team member sections above.
The league does fill up so if you want to play please sign up as early as possible. All registrations are accepted on a space available basis. Payments are not refundable unless the league is full. Please return this registration form and a signed waiver form with a check made payable to Coed Jewish Sports.

Registration Fees
The Summer league registration fee is $105.    (see early registration discount below)
The Fall league registration fee is $69.00.    (see early registration discount below)

Early Registration & Student Discounts
Summer registrants may deduct $10 from the Summer fee if they sign up early, by April 1.
Fall registrants may deduct $5 from the Fall fee if they sign up early, by August 1.
Full-time (undergrad and grad) students may deduct $20.00 from the summer fee (photocopy of student ID is required).
You may sign up for both the summer and fall softball seasons at the same time and subtract an additional $5.00 discount from the total fee.

WAIVER AND RELEASE OF LIABILITY  (READ BEFORE SIGNING)
In consideration of being allowed to participate in any way in the Coed Jewish Sports Softball League, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and 

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and 

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS COED JEWISH SPORTS their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners, and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
